Extension Configuration options are defined in extconftemplate.txt and stored as a serialized array in typo3conf/localconf.php.

$confArray = unserialize($GLOBALS['TYPO3_CONF_VARS']['EXT']['extConf']['cal']);
$locationStructure = $confArray['useLocationStructure'];
